After some research, it looks like the change in NA28 to add ουχ, only found in a few versions and in no extant greek manuscript, was with the inclusion of the Editio Critica Maior (ECM) for the catholic epistles. This new approach to textual criticism relies on a data analysis tool called Coherence-Based Genealogical Method (see attached article for more detail). It’s not clear whether this particular change was driven by the new methodology or the editors’ override.
